Exosomes vs Stem Cells: Not the Same Thing

Exosome treatments have spread rapidly through clinics, often positioned as the newer, better option. They are not stem cells, and the difference is not a technicality.

What an exosome is

Cells release tiny membrane-bound packages containing proteins, lipids and genetic material such as microRNA. These are extracellular vesicles, commonly called exosomes. They are how cells send signals to each other.

The key distinction: an exosome is a package released by a cell, not a cell. It is not alive. It cannot divide, cannot respond to its environment, and cannot become tissue.

The logic behind them

It is not unreasonable. If MSCs work mainly by releasing signalling molecules rather than by becoming new tissue — which is what current evidence suggests — then delivering the signals directly, without the cells, is a coherent idea. Exosomes are also easier to store, ship and standardise.

That is a legitimate research direction. It is not the same as an established treatment.

The regulatory position

No exosome product has FDA approval for any use. The FDA has issued specific public warnings about unapproved exosome products, and has taken enforcement action against distributors. Serious adverse events, including bacterial infections, have been linked to unapproved exosome products administered at clinics.

Exosome products are regulated as biological drugs. Clinics selling them are selling an unapproved drug.

The evidence

Thinner than for MSCs, and MSC evidence is already weak for most marketed uses.

Preclinical work in cells and animals is genuinely interesting across several conditions. Human randomised controlled evidence for the indications sold in clinics — joints, skin, hair, neurological conditions — is largely absent.

The quality control problem

This is the sharpest concern.

There is no standardised, FDA-supervised manufacturing for these products, and no agreed potency assay. Independent analyses of commercially sold exosome preparations have found wide variation in particle counts and content, with some products containing far less than claimed.

With a cell product you can at least ask for a viability count. With exosomes, most clinics cannot tell you what is in the vial, because the supplier cannot either. You are buying an unlabelled preparation from an unregulated supply chain.

Questions worth asking before an exosome treatment

  • Who manufactures this, and under what quality standards?
  • What is the particle count and characterisation for this lot, and may I see it?
  • How is sterility tested?
  • Which randomised controlled trial supports this for my condition?
  • Are you aware the FDA has issued warnings about unapproved exosome products?

The last question is not confrontational. It tests whether the clinic is informed, or hoping you are not.

Where it is heading

Exosome and extracellular vesicle research is one of the more promising areas in regenerative medicine, and it is plausible that approved products will eventually exist. Being promising in research is not the same as being ready to sell, and the current commercial market is running well ahead of the science.
