Panama

Panama has the highest profile in this sector, largely because some clinics there operate under research-oriented frameworks that permit cell therapy the FDA would not allow. Marketing leans heavily on this framing.

Positioned as premium, priced accordingly. English is widely spoken and the US dollar is the currency, which removes friction for American patients.

The thing to check: whether "operating under a research framework" means an actual registered study with ethics oversight and published results, or a regulatory category that permits sale. Ask for the registration identifier and the ethics committee. The answer distinguishes the two.

Colombia

INVIMA is Colombia’s regulator and is generally regarded as more active than some regional counterparts. Medellín and Bogotá have well-developed private healthcare sectors with genuinely good hospitals.

Usually the cheapest of the three. The sector is newer, which means less established reputation on both the good and bad ends of the range — more variance, in either direction.

Costa Rica

The longest-established general medical tourism destination of the three, with a strong reputation in dentistry and cosmetic surgery, and good infrastructure serving international patients.

Regulation of cell therapy tightened following earlier action against unauthorised stem cell operations. Fewer clinics operate there now than at the sector’s peak, which arguably reflects better oversight rather than less availability.

What actually matters

The country is a weak signal. Within each of these countries there are clinics we would consider and clinics we would not, and the gap within a country is far wider than the gap between countries.

What matters, in order:

  1. Can you verify the physician’s licence independently?
  2. Will they disclose cell type, source, dose and viability, with lot documentation?
  3. Is there a registered protocol and genuine ethics oversight?
  4. Is pricing itemised in writing before you travel?
  5. What is the written answer on complications after you fly home?
  6. Are there independent reviews the clinic does not control?

A clinic in Colombia that answers all six beats a clinic in Panama that answers two, regardless of positioning.

On "research framework" marketing

Several clinics across these countries describe their treatment as part of a study or research programme. This is sometimes accurate and sometimes a regulatory convenience. The test is simple: a real study has a registry identifier, an ethics committee you can name, a protocol, and an intention to publish — and does not usually charge participants $20,000. Ask for all four.

Practical notes

Budget for flights, at least one companion, several nights’ accommodation, ground transport, and possible return visits. Check whether your travel insurance excludes complications from elective or unapproved procedures — many policies do.

Take complete records and imaging with you, and bring a full copy of everything home, including exactly what was administered and in what dose. Your physician at home will need it.
