Stem Cell Therapy for Anti-Ageing and Longevity: The Evidence

Longevity clinics sell stem cell infusions at $10,000–$40,000, sometimes as annual programmes. The pitch is compelling: replenish declining cells, reverse biological age, extend healthy lifespan.

There is no human evidence that any of this happens.

What the evidence shows

There are no randomised controlled trials demonstrating that stem cell infusions extend human lifespan, reverse biological ageing, or improve health outcomes in healthy adults. No such product is FDA approved for anti-ageing or wellness.

What exists is early work on cellular senescence and ageing biology, some of it genuinely interesting, almost all of it in cells and animals. Interventions that extend lifespan in mice have a long history of failing to do the same in humans.

The gap between that research and a $25,000 infusion sold today is the entire commercial proposition.

What clinics measure instead

Because they cannot show lifespan extension, longevity clinics measure proxies.

"Biological age" tests. Usually epigenetic clocks based on DNA methylation. Real research tools, but individual results are noisy, vary substantially between tests, and are heavily influenced by recent sleep, illness, and lifestyle. A reported drop after treatment is at least as likely to reflect measurement variability as biological change. These clocks were developed to study populations, not to certify that one person got younger.

Inflammatory markers. A reduction in CRP is a biological signal, not a clinical outcome. Nobody has shown that lowering it by infusion extends life.

How patients feel. Energy, sleep, mood and libido are highly responsive to expectation, attention, and the effect of having done something decisive about ageing. This is the strongest placebo territory in medicine.

Why this is the hardest category to evaluate honestly

There is no disease to track. In knee osteoarthritis you can at least measure pain and function against placebo. For "ageing" in a healthy person, there is no endpoint short of decades of follow-up. That absence of a measurable outcome is exactly what makes the category commercially attractive — nothing can be shown not to have worked.

The risks are the same

Being healthy when you start does not reduce the procedural risk. Infection, clots, and immune reaction remain possible, and a healthy person taking a medical risk for no established benefit has a worse risk-benefit ratio than a sick person taking the same risk hoping for relief.

There is also the financial dimension: annual programmes at $20,000 or more, sold to people with no condition, indefinitely.

What actually affects healthy lifespan

None of this is proprietary and none of it costs $25,000:

  • Not smoking — the single largest modifiable factor
  • Regular physical activity, including resistance training, with strong evidence for cardiovascular and all-cause mortality
  • Sleep — consistent adequate duration
  • Diet quality and maintaining a healthy weight
  • Limiting alcohol
  • Blood pressure, lipid and glucose control — decades of trial evidence
  • Vaccination and age-appropriate screening
  • Social connection — associated with mortality effects comparable to major physical risk factors

An hour of resistance training three times a week has more evidence behind it for healthy lifespan than every anti-ageing infusion sold this year.

If you are still interested

Look for registered trials on ClinicalTrials.gov rather than commercial programmes. Ask any clinic for the randomised controlled trial showing benefit in healthy adults, and note what arrives. Be particularly cautious about programmes structured as recurring annual treatments — that structure serves the clinic’s revenue model, not a clinical rationale.
